Appointments
Please call us at (360) 715-8686 to make your initial appointment. There is often a wait list, so you may want to call as soon as possible. If you are unable to make it to your appointment, we ask that you please let us know 24 hours in advance so we have the opportunity to fill the visit.
Insurance
We will be happy to bill your insurance for you. We will need a copy of your insurance card and the written prescription from your doctor.
Please call the office if you have any questions regarding the referral process. Please be aware that you are responsible for any deductibles, co-pays and non-covered services. Co-pays are due at time of service. We accept most insurance companies, but please see FAQ for a complete list. We would be happy to call your insurance company for you if you are still not sure about your coverage.
What to bring to your first appointment
Download the New Patient Forms (at the bottom of this page) and print them out. Please complete them and bring with you or fax completed forms to us at (360) 715-1680. Arrive 10-15 minutes early to allow time to complete appropriate paperwork.
Please wear loose fitting, comfortable clothing to your physical therapy visits. Wearing a pair of shorts is helpful if any lower extremity evaluation needs to be completed.
